Volunteer Home Care is seeking a compassionate PRN Physical Therapist (PT) to provide one-on-one home health services throughout the Dyersburg area. What You'll Do
As a Home Health Physical Therapist, you'll help patients regain strength, mobility, and confidence while allowing them to recover safely at home.
Responsibilities include:
- Perform comprehensive physical therapy evaluations
- Develop individualized treatment plans based on patient goals
- Improve strength, balance, mobility, endurance, and functional independence
- Provide gait training and fall prevention education
- Recommend and educate patients on the proper use of assistive devices
- Collaborate with physicians and the interdisciplinary care team
- Educate patients and caregivers on exercises and home safety
- Document evaluations, treatments, and patient progress in the electronic medical record (EMR)
- Help reduce hospitalizations and improve patient outcomes through skilled therapy interventions
Qualifications
Required
- Graduate of an accredited Physical Therapy program
- Current Tennessee Physical Therapist (PT) license
- Current driver's license and reliable transportation with automobile insurance
- Current CPR certification (or willingness to obtain upon hire)
- Strong communication, documentation, and clinical decision-making skills
- Ability to work independently in patients' homes while collaborating with an interdisciplinary team
- Commitment to maintaining patient privacy and HIPAA compliance
Preferred
- Home Health experience
- Experience with Medicare documentation
