What You'll Do
As a Home Health RN, you will provide skilled nursing care in patients' homes while coordinating with physicians and the interdisciplinary care team.
Responsibilities include:
- Completing skilled nursing visits according to physician, nurse practitioner, or physician assistant orders
- Performing patient assessments and developing individualized care plans
- Educating patients and families on medications, disease management, and recovery goals
- Monitoring patient progress and communicating changes in condition to providers
- Documenting care accurately using an electronic medical record (EMR)
- Collaborating with therapists, aides, and care team members to achieve positive patient outcomes
- Participating in team meetings, patient care conferences, and quality improvement activities
- Participating in an on-call rotation
Qualifications
Required:
- Current Tennessee Registered Nurse (RN) license
- Current CPR certification
- Valid driver's license
- Reliable transportation and current auto insurance
Preferred:
- Previous Home Health RN experience
- Experience with OASIS documentation and Medicare home health requirements
