Registered Nurse (RN) – Home Health Responsibilities
The Home Health RN provides skilled nursing services in patients’ homes while coordinating care with physicians and the interdisciplinary team.
Responsibilities include:
- Provide skilled nursing visits in the home setting according to physician, nurse practitioner, or physician assistant orders
- Perform patient assessments and develop nursing care plans
- Educate patients and families on medications, disease management, and recovery goals
- Monitor patient progress and report changes in condition to providers
- Document all patient care using the electronic medical record (EMR)
- Collaborate with therapists, aides, and case managers to support patient outcomes
- Participate in team meetings and patient care conferences
- Participate in the on-call rotation

Required:
- Active Registered Nurse (RN) license – Tennessee
- Current CPR certification
- Valid driver’s license
- Reliable transportation and current auto insurance
