What are the responsibilities and job description for the Residential Case Manager position at Volunteer Behavioral Health Care System?
EXEMPLARY DUTIES / RESPONSIBILITIES:
1. Screen consumers referred from RMHI for Supervised Residential services and housing. (If you propose to also use this job title at Greer, take out the word Supervised above)
2. Conduct and document all required assessments for all consumers within 14 days of admission to the residential facility.
3. Complete, with consumer, a Service Plan within 14 days of admission for all consumers, based on the consumer needs as documented in the assessment process.
4. Educate and link the consumer to community resources that may assist the consumer in meeting his/her needs.
5. Assist the consumer in obtaining and maintaining benefits and assist the consumer is accessing services.
6. Complete with the consumer within 14 days a realistic Crisis Action Plan, plus PCP Communications and Emergency Contact documents for all consumers.
7. Maintain a caseload of at 8 consumers.
8. Conduct and document at least four case management visits with each consumer each month.
9. Conduct and document at least one community outing each week for all assigned residents.
10. Conduct daily living/assistance training at least three (3) times a week for all assigned residents and document.
11. Conduct education/training at least two (2) times a week for all assigned residents and document.
12. Completed Continued Stay Reviews and submit to Director Residential Services NLT assigned suspense date.
13. Maintain and update required documentation as required by Policy and Procedure.
14. Maintain all required training.
15. Attend 1 hour Annual and 4 hours Risk Area Corporate Compliance Training.
16. Attend and staff consumers with attending psychiatrist at each consumer’s scheduled appointment.
17. Ensure all prescriptions or prescription changes are turned in to the pharmacy with 24 hours of the change.
18. Coordinate and attend with the consumers all visits to the Primary Care Physician and to complete any lab work ordered by Psychiatrist.
19. Assist the consumer in the development and coordination of consumer’s discharge plan.
20. Refer consumer to outpatient case manager two weeks prior to consumer’s discharge from the Supervised Residential Facility.
21. Coordinate daily with the residential staff and the Residential Services Coordinator.
22. Attend annual Board and Care Training provided by the East Tennessee Office of Licensure, TN Dept of MHDD.
23. Coordinate all changes to treatment and care with the consumer’s conservator, as required.
24. Provide required documentation of compliance to the Mandatory Outpatient Treatment (MOT) Coordinator for all consumers placed in the residential facility under a MOT order.
25. Immediately notify the MOT Coordinator, Center Director and Residential Services Coordinator the consumer’s name and circumstance of any non-compliance with terms of the MOT order.
EDUCATIONAL/ EXPERIENCE:
Minimum of a Bachelor’s degree in a health-related field of counseling, psychology, social work or other behavioral sciences and at least two (2) years of paid work experience in the behavioral health setting preferred. Must have a valid Driver’s License with a clean driving record.
