- Conduct patient evaluations and develop individualized treatment plans
- Provide therapeutic exercises, functional training, and pain management interventions
- Educate residents and families on safe use of equipment and exercise techniques
- Collaborate with interdisciplinary care teams to promote resident progress and independence
- Monitor patient outcomes and adjust treatment plans as needed
- Complete accurate and timely documentation and billing
- Maintain compliance with all state and federal regulations
- Licensed Physical Therapist in Oregon or eligible for Oregon licensure
- Graduate of a CAPTE-accredited Physical Therapy program
- Strong communication, clinical reasoning, and problem-solving skills
- Ability to work collaboratively in a team-based environment
- Skilled nursing or post-acute experience preferred, but not required
