Key Responsibilities
- Coordinate and facilitate the admissions process for new patients
- Ensure all documentation is accurate, timely, and compliant with regulatory standards
- Provide clear communication and compassionate support to patients and families during admission
- Collaborate effectively with physicians, care teams, and support staff
- Assist with assessments to determine appropriate level of care
- Promote a customer-focused experience from initial contact through onboarding
- Participate in implementing and supporting new program initiatives
- Licensed Practical Nurse (LPN) in good standing with the state
- Experience in admissions, case management, or intake preferred
- Strong written and verbal communication skills
- Ability to work independently and within an interdisciplinary team
- Exceptional organizational and time-management skills
- Creative problem-solver with a “can-do” attitude
