What You Will Do

• Oversee an interdisciplinary team of community-based clinicians that deliver quality patient care from admission to discharge.

• Manage, empower, and motivate team members in a dynamic work environment.

• Proactively execute administrative functions to achieve seamless patient care including the review of clinician caseloads, staffing needs, recruitment process, and assigned scheduling of team members.

• Use and learn tools and systems, problem solve new and different situations, and escalate issues timely to leadership to achieve organizational success and improved clinical outcomes.

Licenses and Certifications:

License and current registration to practice as a Registered Professional Nurse, Physical Therapist or Occupational Therapist or Speech Language Pathologist licensed in New York State required

Education:

Bachelor's Degree in Nursing or Physical Therapy from a program approved by the New York State Department of Education or equivalent work experience required orMaster's Degree in Nursing or Physical Therapy or equivalent work experience required

Work Experience:

Minimum of three years nursing experience and one year home health experience required orMinimum of three years of physical therapy experience requiredClinical management experience preferred
