Role Description The Home Health Occupational Therapist is a full-time, on-site role based in Hanover, PA, providing in-home occupational therapy services within the agency’s service area. This role includes evaluating clients’ functional status, developing individualized treatment plans, and implementing therapeutic interventions to improve independence in daily living activities. The Occupational Therapist will educate clients and family members on home-based exercises, adaptive techniques, and use of equipment to promote safety and functional outcomes. Daily responsibilities also include documenting clinical findings, collaborating with interdisciplinary team members, and coordinating care plans with physicians and other healthcare professionals. The role involves traveling to client homes, maintaining professional communication, and supporting the agency’s commitment to high-quality, patient-centered care.
Qualifications
- Strong clinical skills in Occupational Therapy, including assessment, treatment planning, and intervention across various conditions.
- Foundational knowledge of Medicine and Rehabilitation principles to support evidence-based, multidisciplinary home health care.
- Effective Communication skills for interacting with clients, families, and interdisciplinary team members in diverse home environments.
- Current state licensure as an Occupational Therapist (or eligibility to obtain licensure) and graduation from an accredited Occupational Therapy program.
