Role Description
This is a full-time, role for a Home Health Licensed Practical Nurse (LPN) located in Hanover, PA. In this role, the licensed practical nurse will provide professional nursing services to patients in their homes, support comprehensive care plans, and monitor patient conditions. Additional responsibilities include assisting with medication administration, wound care, and collaborating with the interdisciplinary team to ensure optimal patient outcomes. The nurse will also educate patients and their families on managing health conditions at home.
Qualifications
- Current and valid Licensed Practical Nurse (LPN) certification
- Proven skills in nursing and patient care to deliver safe and effective medical treatments
- Experience in home care settings, with a focus on providing personal and compassionate care
- Strong knowledge of fundamental medical principles and hands-on experience in administering medical interventions
- Effective communication and interpersonal skills to collaborate with patients, families, and the healthcare team
- Ability to work independently, manage time effectively, and adapt to varying patient needs
- Reliable transportation and a valid driver’s license for home visit travel
- CPR certification preferred
- Prior experience in a similar role or a home health care environment is a plus
