RN Intake Summary:
Main Responsibilities:
- Coordinate with clinical manager and physicians' offices to ensure proper clinical care of our patients
- Communicate with MD offices with lab results and changes in patient status
- Answers incoming phone calls, gathering and processing patient information from various sources for accurate and comprehensive initial assessment of referrals.
- Obtains necessary referral information to assist with the transition to homecare, including demographics, diagnoses, insurance information, physicians' orders, available caregivers, F2F documentation, etc.
- Assist in the scheduling of patient services
