What You'll Do:
As a Home Health Physical Therapist, you'll:
• Conduct initial patient assessments in the home setting
• Develop and implement individualized treatment plans
• Coordinate care with physicians and healthcare teams
• Promote mobility, independence, and healing through compassionate, hands-on care
• Use our proven case management model to guide patient outcomes and support families
What You'll Need:
• Active Florida Physical Therapist license
• Minimum 2 years of post-graduate experience preferred
• Orthopedic or home health experience is a plus
• Passion for patient-centered care and commitment to quality outcomes
