Private Duty Home Health Client Coordinator -

Our ideal candidate will possess excellent communication skills while providing the highest level of customer service to internal and external customers. Main Responsibilities:
- Answers incoming phone calls, gathering and processing patient information from various sources for accurate and comprehensive initial assessment of referrals.
- Obtains necessary referral information to assist with the transition to homecare, including demographics, diagnoses, insurance information, physicians' orders, available caregivers, F2F documentation, etc.
- Customer Service and Concierge Skills that uses compassion, experience.
