RN - Worcester, Shrewsbury, Boylston, W. Boylston - VNA Care

VNA Care is currently hiring RNs to join our team that covers visits in Worcester, Shrewsbury, Boylston and West Boylston. The ideal candidate will provide quality care to patients in their homes, ensuring they receive the necessary medical attention and support.

Salary range: $86,000 - $129,00 for a full-time schedule, would need to adjust for a part time schedule.

Schedules: Work three to five days, 8a-2p or 8a-4:30p, Monday - Friday

Qualifications
- Must hold a valid MA RN License in good standing
- Minimum of 1 year of med/surg experience; acute care experience is beneficial
- Ability to work with compassion, organization, and excellent multitasking skills
- Willingness to travel within a designated territory

Responsibilities
- Conduct home visits to assess patient needs and provide necessary care
- Utilize Case Management, Oasis E documentation, Time Management, and EMR system, Home Care Home Base
- Collaborate with patients, families, and interdisciplinary team members to develop and implement care plans
- Ensure accurate documentation of patient progress and treatment plans
