We are looking for talented individuals to join an amazing team at VNA Care! We have an opportunity for RN's to join our Hospice & Palliative Care Team!
Overview: VNA Hospice & Palliative Care's Mission is to provide the right care with kindness and compassion every day for every person we serve. As a member of the interdisciplinary team, the Hospice RN works under the supervision of a Clinical Manager and Medical Director in the provision of skilled nursing care.
Hospice Triage RN Responsibilities include:
- Receives and responds to agency telephone calls.
- Communicates accurate information reflecting knowledge of agency services, Hospice regulations and agency policies and procedures.
- Obtains and assesses clinical information regarding patients condition and responds accordingly and timely to accepted standards of nursing practice.
- Responds to patients needs for direct care by planning and coordinating the care with extended hours staff.
- Accepts telephone orders from physicians, confirms in writing according to agency policy, and routes information to appropriate care giver and team including notifying staff in the field as necessary.
- Works with central intake to accept referrals for agency services
- Maintains current knowledge of Hospice and Medicare/Medicaid regulations as well as Hospice licensure.
Education:
Graduate of a certified RN program required; Bachelor of Science Degree in Nursing preferred.
Licensure:
- Licensed to practice in Massachusetts
- Must be able to travel to Massachusetts VNA Care office location on occasion.
- Minimum of 2 years general med/surg experience with a minimum of 1 year in an acute care setting.
- Hospice experience is required.
- Demonstrates a can-do attitude and the ability to respond appropriately to the needs of patients and families in varied environments.
- Strong punctuality and timeliness in terms of shift attendance.
- Proficient interpersonal relations and communication skills.
- Home care experience required.
