VNA Care: RN Case Manager (Full-Time)

Schedules available: Monday- Friday, 40 hours from 8a-4:30p.
Salary range: $93,000 - $125,000
Training and Support: We offer a fully paid extensive Home Health orientation program.
Requirements:
Compassionate, enjoy working with patients and their families, organized and ability to multi-task.
- Good standing MA RN License
- 1 year of med/surg experience; 1 year of acute care experience helpful
- Willingness to travel within a territory.
