Job Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for a local healthcare facility. Responsibilities
- Conduct comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations.
- Administer medications and injections accurately following prescribed protocols while maintaining strict adherence to HIPAA and infection control standards.
- Manage IV infusions, catheter care, tube feeding, and ventilator support as required.
- Document all patient interactions and treatments thoroughly in electronic health records (EMR/EHR systems such as Epic or Cerner).
- Assist with diagnostic evaluations including specimen collection/processing, laboratory testing, and medical imaging coordination.
- Provide specialized care for patients with dementia, Alzheimer's disease, memory care needs, or behavioral health conditions.
- Support discharge planning and case management activities to ensure seamless transitions of care.
- Educate patients and families on health coaching, medication management, and post-acute care plans.
- Maintain sterile techniques during procedures like suturing or spinal taps and ensure infection control protocols are followed diligently.
Skills
- Proficiency with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks; familiarity with medical terminology and anatomy knowledge.
- Skilled in patient assessment techniques including vital signs monitoring and physical examinations.
- Ability to perform venipuncture, phlebotomy, catheterization, sterile processing techniques, and medication administration including injections.
- Strong understanding of infection control practices and aseptic techniques.
- Experience managing complex cases involving acute pain management, infectious disease care, or trauma (Level I & II Trauma Center experience).
- Knowledge of specialized procedures such as cardiac catheterization or gastrostomy tube management is advantageous.
- Excellent communication skills for working with patients of all ages—including children—and collaborating effectively within healthcare teams.
- Ability to work independently while maintaining high standards of patient safety and confidentiality under HIPAA regulations.
