What You’ll Do
- Perform comprehensive patient assessments, including detailed medical histories, physical exams, and appropriate diagnostic testing
- Diagnose and manage acute and chronic conditions using evidence-based and functional medicine principles
- Prescribe medications and therapies tailored to each patient’s individual needs
- Educate and empower patients through guidance on nutrition, lifestyle changes, disease prevention, and long-term wellness
- Order, interpret, and follow up on laboratory tests, imaging, and other diagnostic tools
- Collaborate closely with our providers and healthcare team to deliver coordinated, high-quality care
- Document patient encounters accurately and efficiently in our electronic health record (EHR) system
- Participate in clinic initiatives focused on improving patient outcomes, clinic flow, and overall performance
- Stay current with continuing education, clinical guidelines, and best practices in patient-centered care
What We’re Looking For
- Master’s degree in Nursing from an accredited Nurse Practitioner program
- Current state licensure as a Nurse Practitioner
- Board certification in Family Medicine, Adult-Gerontology, or a related specialty
- Strong clinical judgment with the ability to work both independently and collaboratively
- Excellent communication skills and a genuinely compassionate approach to patient care
- Strong time-management skills and comfort caring for a diverse patient population
- Active DEA license
