Key Responsibilities

Perform comprehensive assessments for new admissions and acute issues in both subacute and long-term care patients

Prevent avoidable hospital transfers through early evaluation and timely intervention

Collaborate and communicate effectively with primary care providers and specialists

Participate as an active member of the interdisciplinary care team
Qualifications

Previous RN experience required — New NP graduates are welcome to apply!

Strong critical thinking and assessment skills

Ability to thrive in a fast-paced environment

Excellent written and verbal communication skills

Comfortable with electronic documentation and computer systems
Compensation
$120,000 – $170,000 per year
(Full Time Base Salary Productivity Bonus)
$65.00-$75.00 per hour
