Job Description
Vitality Hospice & Palliative Care is seeking a compassionate and experienced Admissions Registered Nurse (RN) to join our clinician-owned and operated hospice agency. Key Responsibilities
- Complete comprehensive hospice admissions and patient/family assessments
- Ensure accurate documentation and appropriate care planning
- Provide direct patient care as needed, including medication administration and treatments
- Collaborate with physicians and the interdisciplinary team to develop individualized plans of care
- Serve as a liaison between patients, families, and healthcare providers
- Educate patients and caregivers on disease progression, symptom management, and hospice services
Qualifications
- Active Registered Nurse (RN) license (required)
- 1 year of skilled nursing experience preferred
- Hospice or admissions experience preferred (not required)
- Willingness to travel within assigned service areas
- Strong communication, organizational, and interpersonal skills
- Valid driver's license, reliable transportation, and auto liability insurance
