Position is Full Time: The requirement is 48 hours of on call on a weekend and 4 nights during the week- Monday- Friday 5p-8a.

Qualifications:
- Current Pennsylvania Registered Nurse (RN) License
- Minimum one year of Registered Nurse (RN) experience
- Willing to travel to the assigned areas
- Must have a valid drivers’ license with reliable transportation and liability insurance
- Strong communication and interpersonal skills

Responsibilities:
- Conduct thorough assessments of patients and their families to ensure proper care planning.
- Provide direct patient care, including monitoring, administering medications, and performing treatments.
- Collaborate with the healthcare team to develop individualized care plans.
- Facilitate communication between patients, families, and healthcare providers.
- Educate patients and families about disease management, treatment, and self-care techniques.
Qualifications:
- Valid nursing license and relevant certifications in hospice care
- 1 years of prior skilled nursing experience preferred
- Willing to travel to the assigned areas
- Strong communication and interpersonal skills
- Valid driver’s license, auto liability coverage, and reliable transportation
