We are seeking a dedicated Per Diem Hospice LPN to provide high-quality, patient-centered care to individuals receiving end-of-life services in their homes or facilities. This role offers flexibility, autonomy, and the opportunity to make a meaningful impact with every visit.
What You’ll Do
- Provide skilled hospice nursing care under the direction of the RN Case Manager and clinical leadership.
- Deliver care according to the patient’s individualized plan of care.
- Perform assessments and report changes in condition promptly.
- Support pain and symptom management while ensuring patient comfort.
- Educate patients, families, and caregivers on care needs and self-care strategies.
- Document visits and care provided accurately and in a timely manner.
- Collaborate with the interdisciplinary team to ensure seamless, holistic support.
- Assist patients with ADLs as needed, promoting dignity and independence.
- Participate in on-call rotation as available (optional based on per diem structure).
Qualifications
- Active and unencumbered LPN license in the state of practice.
- Minimum 1 year of nursing experience preferred (hospice experience a plus).
- Understanding of hospice philosophy and the needs of terminally ill patients.
- Strong communication, assessment, and documentation skills.
- Reliable transportation, valid driver’s license, and current auto insurance.
- Ability to work independently and provide compassionate, professional care.
