Consult Coordinator/Grievance Nurse:

CONSULT COORDINATOR/TELEHEALTH COORDINATOR LICENSED PRACTICAL NURSE RN or LPN POSITION SUMMARY

The Consult Coordinator helps facilitate medical consultations and appointments for patients who require specialized care beyond the facility's capabilities. They coordinate with outside healthcare providers, schedule appointments, arrange transportation, and ensure that necessary medical records are prepared. Overall, the role of a Consult Coordinator in a correctional facility involves a combination of clinical care, coordination, and administrative tasks to provide essential healthcare services to inmates while adhering to the unique challenges of a correctional environment.

The Telehealth Coordinator help facilitate virtual medical consultations and appointments for patients who require specialized care beyond the facility's capabilities. This position will monitor patients from intake to the facility and ensure continuity of care leading up to and following release into the community by advocating for patient needs while acting as a liaison between the multiple systems impacting the lives of offenders to provide a proactive and collaborative approach.

CONSULT COORDINATOR/TELEHEALTH COORDINATOR RESPONSEBILITIES:

• Consult Coordinator/Grievance Nurse meets with inmates/patients regarding complaints of medical care and facilitating resolutions with the patient and medical team.


• Ability for tracking & trending data for presentation to a committee.

CONSULT COORDINATOR/TELEHEALTH COORDINATOR REQUIREMENTS:

•  Graduate of an accredited School of Nursing


•  Licensure as a Registered Nurse or Licensed Practical Nurse in the state of employment


•  Possesses an active CPR certification


•  Remains knowledgeable about specific state laws and regulations governing practice


•  Satisfactory completion of initial and annual clinical competencies to demonstrate aptitude as assigned by role


• Must have the ability to work independently, take initiative, and have good judgment and problem-solving skills.

• The  Telehealth Coordinator maintains accurate and up-to-date medical records for patients receiving specialized care. This involves documenting medical histories, test results, treatment plans, and communication with external healthcare providers.


• The  Telehealth Coordinator acts as a liaison between the correctional facility's medical team and external healthcare providers. They communicate medical needs, coordinate information exchange, and ensure seamless collaboration for patients' medical care.


• The RN Telehealth Coordinator acts as a liaison between the correctional facility's medical team and external healthcare providers. They communicate medical needs, coordinate information exchange, and ensure seamless collaboration for patients' medical care.


• Alongside clinical duties, RN Telehealth Coordinators may have administrative tasks such as scheduling appointments, maintaining records, and coordinating with external healthcare providers.

Part-Time Day Shift
