Your Role: Registered Nurse (RN)
As a Registered Nurse at VitalCaring, you'll do more than provide care - you'll lead it.
You will deliver skilled, patient-centered care that prioritizes comfort, dignity, and outcomes, while building meaningful relationships with patients and families. This is a role for a clinician who values both clinical excellence and human connection.
What You'll Do
- Conduct comprehensive patient assessments and identify care needs
- Develop and manage individualized plans of care in collaboration with physicians
- Deliver skilled nursing care, including treatments, medication management, and interventions
- Monitor patient progress and proactively communicate changes in condition
- Educate patients and families to support confidence and continuity of care
- Serve as case manager, coordinating care across the interdisciplinary team
- Maintain accurate, timely clinical documentation
- Support and supervise care team members, including home health aides
- Participate in on-call rotation as needed
What You Bring
- Active RN license in the state of practice CPR certification
- Graduate of an accredited nursing program
- Minimum of one (1) year experience as a RN; Home health, Med-Surg or Acue Care experience preferred
- Strong clinical judgment and critical thinking
- Compassionate communication and patient-first mindset
- Reliable transportation and ability to travel within the assigned territory
