What You’ll Do
- Provide direct patient care in the home under the supervision of a Registered Nurse
- Execute individualized care plans and support positive patient outcomes
- Educate patients and caregivers on disease management and self-care
- Document care accurately and timely in the EMR
- Collaborate with physicians and interdisciplinary teams to ensure coordinated care
- Build trusted relationships with patients, families, and care teams
What You Bring
- Active LPN/LVN license in the state of practice
- Current CPR certification
- Minimum of one (1) year nursing experience (home health, community, or med-surg a plus)
- Strong clinical judgment, communication, and organization skills
- A patient-first mindset and passion for meaningful care
- Valid driver’s license, reliable transportation, and auto insurance
