Key Responsibilities

• Provide Essential Supports: Complete agency training or provide proof of experience in home-care tasks, and pass a home-care competency evaluation.
• Maintain a Safe Environment: Ensure a clean and safe home environment for the client, manage household chores, and prepare nutritious meals.
• Offer Companionship: Engage clients in conversation, provide emotional support, and coordinate mentally stimulating activities.
• Assist with Daily Living: Help with bathing, dressing, grooming, medication reminders, and other daily activities.
• Document and Report: Keep thorough notes on services provided, report any changes in the client’s condition, and maintain timely records of home visits.
• Participate in Ongoing Training: Attend periodic educational sessions, care coordination conferences, staff meetings, and agency committee meetings.
• Uphold Standards: Follow infection control precautions, provide basic first aid, and comply with all applicable laws, regulations, and standards.

Qualifications

• At least 18 years of age.
• High school diploma or GED.
• Strong communication skills for interacting with clients, family representatives, and other providers.
• Successful completion of a written competency exam related to the agency PCW training course.

Desired Skills and Abilities

• Knowledge of personal care principles and procedures in a home setting.
• Comfort in handling sensitive family issues and matters related to aging.
• Understanding of nutrition and food service.
• Conscientious, observant, and proactive in documenting services and reporting changes in client health.
• Effective communication skills, both verbal and written.
• Ability to build rapport easily with managers, clients, families, and colleagues.

Special Requirements

• Valid, current state driver's license and access to a reliable automobile or public transportation.
