Responsibilities
- Conduct thorough assessments of patients’ physical conditions, including physiology and anatomy evaluations
- Implement and follow individualized care plans tailored to patient needs
- Collaborate with multidisciplinary teams to coordinate patient care and optimize recovery outcomes
- Educate patients and their families on exercises, mobility techniques, and injury prevention strategies
- Document treatment progress accurately using EMR systems and maintain detailed patient records
- Monitor patient progress and modify treatment plans as necessary to achieve optimal results
- Stay current with advancements in physical therapy techniques and medical terminology relevant to outpatient care
Skills
- Strong knowledge of physiology, anatomy, and medical terminology relevant to physical therapy practice
- Experience in outpatient clinics
- Proficiency with EMR systems for documentation and patient management
- Skilled in discharge planning and developing effective care plans for diverse patient populations
- Expertise in physical therapy for neurological conditions
- Ability to work with geriatric, orthopedic, neurologic, and medically complex patients
- Excellent patient care skills with a compassionate approach toward rehabilitation goals
- Strong communication skills for effective collaboration with the team
