Responsibilities:
- Provide 1:1 compassionate and personalized nursing care to clients in their homes.
- Administer medications and monitor patient health status.
- Assist with daily living activities, including hygiene and mobility support.
- Educate clients and families on health management and wellness practices.
- Document patient progress electronically and communicate changes to the care team
- Ensure a safe and comfortable environment for clients.
Requirements:
- Current RN or LPN license in the state of New Jersey.
- Previous experience in home care or a similar healthcare setting.
- Trach and/or vent experience preferred
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassionate demeanor with a commitment to patient-centered care.
- Reliable transportation and willingness to travel to client locations.
- Flexible availability for full-time or part-time shifts.
