Position: Nurse Practitioner (NP)

We are seeking an experienced Nurse Practitioner (NP) to join a collaborative PACE (Program of All-Inclusive Care for the Elderly) team in beautiful Grand Junction, Colorado.
This role offers the opportunity to provide comprehensive, patient-centered care for seniors with chronic and complex medical needs while working within a highly supportive interdisciplinary care model. If you have experience in Primary Care, Family Medicine, Geriatrics, Internal Medicine, Home Health, or Senior Care, we'd love to hear from you.
Position Highlights

Key Responsibilities
- Provide comprehensive primary care services, including diagnosis and treatment of acute and chronic illnesses.
- Deliver preventive care, wellness services, and urgent medical treatment as needed.
- Conduct health assessments in the clinic, PACE center, and participants' homes.
- Collaborate closely with physicians, nurses, therapists, social workers, and the interdisciplinary care team.
- Develop individualized treatment plans and coordinate ongoing care.
- Make timely referrals to specialists and community providers.
- Support patients and families through compassionate, high-quality care that promotes independence and quality of life.
Required Qualifications
✔ Current and unrestricted Colorado APN license
✔ Prescriptive authority (RXN) in Colorado
✔ Current or eligible DEA registration and controlled substance license
✔ Current CPR/BLS certification
✔ Valid driver's license, reliable transportation, and acceptable driving record
✔ Minimum 2 years of Nurse Practitioner experience
Preferred Qualifications
- Family Nurse Practitioner (FNP) certification
- Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) certification
- Experience in Primary Care, Geriatrics, PACE, Home Health, Hospice, Internal Medicine, or Community Health
- Strong care coordination and chronic disease management experience
