We are looking for Bilingual Chinese Fee-Based Registered Nurses..

Responsibilities include (but are not limited to):

• Obtain Admission Packets from the designated location of the contracted home care agencies. Distribute and explain the Admission Packets to newly admitted patients.
• Conduct comprehensive head-to-toe assessment and environmental evaluation for newly referred patients.
• Review, develop, and implement the plan of care in light of the patient's medical physician and/or the insurer's treatment plans.
• Conduct routine reassessments and PRN visits of patients to assess patients' conditions. Revise plan of care and communicate with patient’s medical physician as needed.
• Provide in-person and/or telephonic clinical guidance, orientation, supervision, and training for paraprofessional personnel.
• Coordinate telephone calls with patient’s physician and other health care providers regarding the patient’s plan of care and complete Medical Order forms
• Facilitate safe discharge planning and document discharge summary from Licensed Home Care Service Agencies
• Perform other tasks required for the routine operation of patient services and the nursing department of the home health care agency.
• Respond to case assignment emails within the same business day of receipt.
• Complete electronic (paperless) documentation with company laptop provided.

Requirements:

• Licensed and registered in NYS as a Registered Professional Nurse
• Must be consistently available and able to cover at least one borough/county: Manhattan (Urgent Need), Brooklyn, Queens, Staten Island, Bronx, and Long Island
• Legally authorized to work in the United States
• Graduate of an accredited school of nursing with a diploma, baccalaureate, or master's degree
• Working knowledge of NYSDOH rules and regulations
• Strong problem-solving, organizational, written, communication, and interpersonal skills.
• Proficiency in basic computer and smartphone programs
• Self-motivated and reliable work ethic
• Verifiable personal and professional references required.
• Regular access to the Internet/email and a mobile phone is a must.
• A driver's license and a vehicle are a plus.
• Home care experience is preferred, but new grads are welcome!

Language Requirements:

• English-Chinese Bilingual (Cantonese and/or Mandarin)

Pay Rate: (Salary)

Fee-for-visit structure:

• Initial Visits: Rate: $90-110
• Revisits: $70-90 (Depends on patient’s location)
• Specialty Case: $20 in addition to the Initial/Revisit rates
