About the Opportunity
Kabila Wellness, operating as Vital Care of Shelton, is a growing home infusion and specialty pharmacy serving patients throughout Connecticut.
We are seeking an experienced Per Diem Pharmacist to provide pharmacy coverage and support our home infusion operations. Because sterile compounding quality and regulatory compliance are central to our operation, demonstrated knowledge of USP <797> and sterile compounding practices is essential for this position.

Key Responsibilities
- Provide pharmacist coverage for home infusion pharmacy operations as needed.
- Perform pharmacist duties consistent with Connecticut law, applicable pharmacy regulations and organizational policies.
- Review and verify prescriptions and medication orders for accuracy, appropriateness and completeness.
- Oversee sterile compounding activities within the pharmacist's scope and responsibilities.
- Support adherence to USP <797> requirements and applicable sterile compounding standards.
- Ensure appropriate documentation related to sterile compounding activities, including required logs, records and quality-control documentation.
- Review compounded sterile preparations for accuracy and compliance with applicable requirements.
- Support appropriate beyond-use dating, storage, labeling and handling practices.
- Participate in ongoing quality assurance and continuous compliance activities.
- Identify potential compliance or patient-safety concerns and escalate them promptly.
- Support implementation and maintenance of pharmacy policies and SOPs.
- Participate in regulatory and accreditation readiness activities as appropriate.
- Collaborate effectively with pharmacy technicians, nurses, prescribers, management and other members of the care team.
- Provide patient- and clinician-focused pharmacy support consistent with the standards of a high-quality home infusion organization.
Required Qualifications
- Active pharmacist license in the State of Connecticut in good standing.
- PharmD or BS in Pharmacy from an accredited school of pharmacy.
- Demonstrated knowledge and practical experience with USP <797> and sterile compounding.
- Strong understanding of medication safety, quality assurance and pharmacy regulatory requirements.
- Ability to work independently and exercise sound professional judgment.
- Strong attention to documentation, process discipline and detail.
- Ability to remain effective and exercise good judgment in a fast-moving healthcare environment.
- Excellent communication and collaboration skills.
Highly Preferred Experience
- Home infusion pharmacy.
- Sterile compounding pharmacy operations.
- Specialty pharmacy.
- USP <797> implementation, assessment or remediation.
- Cleanroom and/or compounding aseptic isolator operations.
- Pharmacy accreditation or regulatory inspections.
- ACHC or similar healthcare accreditation.
- Connecticut pharmacy regulatory requirements.
- Quality management, compliance or pharmacy operations leadership.
- Development or implementation of pharmacy SOPs, training programs or quality systems.
