VITAE - Nurse Practitioner Program
The Position
As we continue to grow, we are looking for “SNF-ists,” full-time psychiatric nurse practitioners that will provide care for the geriatric population in a Skilled Nursing and/or Assisted Living setting and bring quality mental health services to those communities.
What You Will Do
As the psychiatric nurse practitioner, you will act as a Skilled Nursing Facility’s leading provider of psychiatric services. You will collaborate with the facility’s interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all of the facility’s residents.
You will work with the entire facility’s patient population - short and long-term residents - overseeing their care with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians.

VITAE - Nurse Practitioner Program
The Position

You will work with the entire facility’s patient population - short and long-term residents - overseeing their care with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians.
As a psychiatric nurse practitioner, you will provide types of care, including but not limited to:
- Evaluation of new-admission visits
- Coordination of discharge as needed
- Monthly stability visits for long term residents
- Evaluations for patients exhibiting changes in condition
- Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease
- Prescribe all necessary medications and treatments
- Facilitate referrals to other healthcare professionals and medical facilities
- Communicate with attending physicians and/or specialists regarding patient care
- Proficiency with documentation and CPT coding
- A healthy work-life balance with flexibility. No need to work overnight shifts or weekends, however as a Vitae provider, you can always take off days during the week and work weekends if you’d like.
- Malpractice insurance and tale coverage for $1MM/$3MM coverage will be provided by Vitae.

Licensure / Certification / Registration:
- Advanced Practice Licensure (APN) with an applicable state license
- PHMNP certification
- Must have an active license and DEA #
- Previous experience in nursing or other medical fields
- Minimum one-year experience in a Skilled Nursing setting (preferred)
- Ability to build rapport with patients and their loved ones
- Ability to thrive in a fast-paced environment
- Excellent written and verbal communication skills
- Strong leadership experience with emphasis on teamwork in a healthcare setting
- Master’s degree required
