Vista Living Care is looking for a compassionate and relationship-centered nurse leader to help support our integrated dementia care model across residential Memory Care and dementia-focused Home Care services.

What You’ll Do
Clinical Leadership
• Resident assessments & care plans
• Medication oversight
• Clinical documentation review
• Dementia-focused caregiver education
• Quality assurance & compliance
Continuity of Care
• Support transitions between Home Care & Memory Care
• Coordinate communication across care settings
• Help guide families through care progression
Team & Family Support
• Collaborate with leadership teams
• Support caregiver coaching & clinical competency
• Participate in interdisciplinary care planning
• Lead family meetings & provide clinical guidance
We’re Looking For Someone Who Has:
- RN or LPN License in New Mexico
- Experience in Memory Care, Hospice, Home Health, or Senior Living
- Strong Dementia Care Knowledge
- Calm, Practical Leadership Style
- Excellent Communication & Emotional Intelligence
- Collaborative & Relationship Focused Approach to Care
