PROGRAM
The Vista Hill Care Connect is a brand-new program which will work with healthcare provers of MediCal patients who need support services and help them make connections. This maybe for resources such as mental health, housing, food assistance, medical transportation, etc. This is a new contract and this position will be on the ground floor to help develop the program and grow the team as demand increases.
POSITION
The Program Manager is responsible for the clinical integrity, assessment, planning and delivery of services to all clients within the program area. The Program Manager is responsible for maintaining relationships with all stakeholders including County, district, community, and family partners and participates in stakeholder forums, meetings, and consultations to ensure the most effective service delivery. The Program Manager also ensures that all program policies and procedures, staffing patterns, physical plant properties, service delivery, program documentation, and billing are consistent with company policies and contracts, as well as any federal, state, county, and partner agency requirements. Examples of tasks include:
- Provide supervision, education, and training and oversees the work product of all staff in daily operations.
- Ensures that program services rendered by staff adhere to the appropriate contractor and licensing standards, procedural requirements, and quality standards.
- Supervise and monitor all aspects of the program to include program development, quality assurance and contract compliance.
- Provide routine audit of all medical records ensuring that records comply with all known documentation standards and procedural requirements.
- Ensures that the Director is notified of all contract compliance issues, is updated on all reports submitted to stakeholders, and is informed of all staff disciplinary issues including measures used to resolve potential issues.
- Ensure that all unusual occurrences, critical incidents, and patient, staff, and stakeholder complaints are thoroughly investigated, documented, and responded to, according to Vista Hill policy, and applicable ethical and legal requirements.
- Ensures that the program is successfully meeting all contract objectives. Develop and implement a plan to address any areas needing improvement.
- Monitors, develops, and reports on Vista Hill and stakeholder performance outcome objectives and ensure that data is current and accurately recorded.
- Work closely with stakeholders and multiple agencies to conduct outreach and develop opportunities.
- Work with Program Director to maintain budget compliance and develop yearly budgets. Monitor program progress towards budget goals.
- Ensure that all electronic record systems are up to date and staff comply with data in a timely manner.
- Ensure that all Privacy Practice standards and ethical practice standards are implemented within the program.
- Hires, trains, schedules, assigns, supervises and evaluates staff.
- Submits monthly program reports as required in a timely manner.
REQUIREMENTS
- Bachelor’s degree in social services or other helping professional required. AOD certification may substitute for Bachelor’s degree in programs with a Substance Use component. Master’s degree preferred.
- Must have minimum of five (5) years’ experience working with the program’s defined service population.
- Must have minimum of two (2) years’ experience working with staff, training and supervising.
- Experience with contract documentation and procedural requirements of the defined scope of work for the contracts to be managed.
- Must be able to work with culturally diverse population.
- Must be free of parole and probation for a period specified by program contract.
- Must be free of substance abuse or psychoactive substances for duration specified by program.
- Must pass pre-employment background check, TB test and drug screening and any other County or State check as required by Program.
- Requires a California Driver’s License and clean driving record.
HOURS
Monday- Friday, 40 hours per week, 8:30 – 5pm.
PAY
Current Vista Hill employees in these job titles for 2024 are paid $33.70 - $36 per hour.
BENEFITS
Employees working over 30 hours a week receive employer-paid benefits, including medical, dental, vision, paid time off (vacation and sick leave), paid holidays, employer-paid retirement contribution and clinical supervision (if applicable). Some programs qualify for student loan forgiveness programs.
Employees who work less than 30 hours a week do not qualify for healthcare benefits.
