What are the responsibilities and job description for the CASE MGR I position at Vista Hill Foundation?

PROGRAM

The Vista Hill Care Connect is a new program which will work with healthcare provers of MediCal patients who need support services and help them make connections. This maybe for resources such as mental health, housing, food assistance, medical transportation, etc. This is a new contract and this position will be on the ground floor to help the Manager grow the team as demand increases.

POSITION

The Case Manager acts as a liaison between referral sources (i.e. School, Social Worker, Probation, Group Homes, Courts, other Treatment Providers, etc.) and the client. The Case Manager is responsible for monitoring client progress and helping the client coordinate the implementation of resources such as health, housing, education, assist with appointments, provide transportation if needed, etc. The Case Manger is responsible for documenting all client contacts within the program guidelines. The role of the Case Manager is to work with the client and support systems to help establish long-term stability, and must be available to work schedules that may include day, evening and weekends to best meet the needs of the population served. Examples of tasks include:

  • Provides case management services to clients including intake, assessment, and referrals.
  • Develop a personalized case management program specific to the client’s needs. Coordinate care with the Program team to ensure client success.
  • Develop and monitor client progress and compliance, meeting with clients as specified by program or individual needs.
  • Offer direct service which may include family engagements, behavioral intervention plan development and implementation, resource linkage and support, and transportation that will support client success.
  • Provide behavioral rehabilitation, education, and training on topics based on program such as behavior management, decision making, anger management, employment, etc.
  • Complete Medi-Cal applications and develop a personalized treatment/recovery program to ensure seamless and smooth transition into treatment program.
  • Provide ongoing client meetings and follow-up through to program completion, including one on one meetings, phone calls and home visits (when deemed necessary by Program Manager).
  • Build and maintains relationships with programs and agencies such as referral sources, schools, probation, Child Welfare, community agencies, employer partner organizations, etc. and utilizes them as part of the multi-disciplinary team.
  • Provide crisis intervention and conflict resolution as needed.
  • Establish weekly case presentations for review at staff meetings. This process includes all new admissions, discharge and referrals within the past week. Participate in case file reviews.
  • Maintain accurate, compliant and timely record for each client including all client contacts, case notes, progress reports, statistics, termination summaries, and discharge plans as required.
  • Ensure the implementation of client confidentiality standards, in line with all of HIPAA, Article 14.

  • Complete and submit all program required reports in timely manner.

REQUIREMENTS

  • Bachelor’s Degree in psychology, sociology, or a related field and one year of experience OR
  • Associate’s Degree in psychology, sociology, or a related field and 3 years of experience.
  • Must offer certain linguistic, gender, cultural and ethnic specific skills: must be able to work with culturally diverse population.
  • Must be free of parole and probation for a period specified by program contract.
  • Must have a valid California driver's license and a clean driving record.
  • Must be free of substance abuse or psychoactive substances for duration specified by program.
  • Must pass pre-employment background check, TB test and drug screening and any other County or State check as required by Program.
  • May require bilingual abilities based on population needs. Bilingual English/Spanish preferred.

HOURS

Monday- Friday, 40 hours per week, 8:30 – 5pm.

PAY

$21.50 - $27.00 per hour

Current Vista Hill employees in these job titles for 2024 are paid $21.50 - $25.17 per hour.
