JOB TYPE: Independent contractor
COMPENSATION RANGE: Dependent on Services Provided
LOCATION: Logan, Utah and surrounding areas
POSITION RESPONSIBILITIES:
As a direct support contractor, your role and responsibilities will vary depending on the
type of service.
Respite Provider:
- Temporarily taking an individual with disabilities into your home, or staying at the
- Maintain the individual's health, safety, and wellbeing.
- Assist with activities of daily living and personal care as needed.
- Support recreational, social, and community-based activities.
- Follow the individual’s support strategies, behavioral supports, dietary needs, and
- Provide transportation when authorized and appropriate.
- Complete required service documentation and communicate relevant information
- Pay rate is negotiated between the regular caregiver and the respite provider. Typical
level and negotiations between the caregivers.
Supported Living Provider:
- Provide individualized support to help people live as independently as possible in
- Assist with daily living skills such as cooking, cleaning, laundry, personal care, and
- Support budgeting, shopping, money management, and other independent living
- Assist individuals with accessing community resources, appointments, activities,
- Provide transportation and community access as authorized.
- Promote self-determination, choice, communication, advocacy, and development
- Support health and safety needs according to the individual’s support plan.
- Complete required service documentation and communicate with the individual’s
- Pay begins at $19.00 per hour and may be more based on the individual’s needs
Day Supports (Individual) Provider:
- Provide individualized daytime support and supervision to people with intellectual
- Assist individuals in developing a community-based activity calendar with
- Assist individuals in developing and maintaining social, communication, self-help,
- Support participation in meaningful community-based activities based on individual
- Encourage independence, choice, self-advocacy, and development of natural
- Assist with personal care and activities of daily living when needed.
- Provide transportation to community activities and appointments when authorized.
- Implement individualized goals and support strategies.
- Maintain required documentation regarding services, activities, progress, and
- Pay begins at $19.00 per hour and may be more based on the individual’s needs
- Must be 21 years of age or older.
- Must have a valid driver’s license, reliable transportation, current and adequate
- Must be able to pass a combination of all or some of the following: state and/or
required by regulation or best practice.
- Must have a reliable computer and internet access or ability to use cellular phone
- Must be able to pass all state-mandated and/or Vista Care-required training courses
