About the Role:
Visium HealthLink seeks LPNs to serve as the Nurse Navigator to an assigned load of up to 500 patients monthly. The Nurse Navigator will monitor patient health data and provide telephonic support. In this role, you'll connect with patients monthly to empower, offer support, educate them on preventative care, and help improve their overall health.
The Nurse Navigator plays a crucial role in guiding patients through the healthcare system, ensuring they receive the appropriate care and resources tailored to their individual needs. This position involves collaborating with healthcare providers, patients, and their families to create personalized care plans that enhance patient outcomes and satisfaction. The Nurse Navigator will also be responsible for educating patients about their health conditions and available treatment options, empowering them to make informed decisions. By effectively coordinating care and addressing barriers to access, the Nurse Navigator aims to improve the overall patient experience and health outcomes. Ultimately, this role is vital in fostering a supportive environment that promotes health and well-being within the community. Greater than 75% of daily time will be spent making outbound patient calls. Work may be on-site, hybrid, or fully remote as required to meet client needs.
Minimum Qualifications:
- Current, active LPN
- 2 years patient care experience.
- HIPAA-compliant office with access to secure high-speed internet.
- Experience in patient care coordination or case management.
- Strong understanding of healthcare systems and resources.
- Ability to thrive in fast-paced environment.
- Desire to contribute to teamwork.
Preferred Qualifications:
- Certification in care coordination or case management.
- Experience working with diverse populations and understanding cultural competency.
- Familiarity with multiple electronic health records (EHR) and healthcare software.
Responsibilities:
- Successfully complete up to 20 calls daily.
- Conduct comprehensive assessments of patients' healthcare needs and preferences within scope of practice.
- Develop and implement individualized care plans in collaboration with healthcare teams.
- Facilitate communication between patients, families, and healthcare providers to ensure continuity of care.
- Provide education and resources to patients regarding their health conditions and treatment options.
- Monitor patient progress and adjust care plans as necessary to meet evolving needs.
