Overview:
VNS Inc. of Southern CT has RN Case Manager positions available for the following areas: Hamden, North Haven and Wallingford. Must have Home Care Experience.
Responsibilities:
* Assesses/reassesses patients and establishes plans of care that are patient specific.
* Collaborates with providers, other disciplines, and communicates all changes promptly.
* Educates and instructs patients/caregivers in their ongoing medical maintenance. Begins discharge planning on admission.
* Oversight of LPNs and Home Health Aides.
* Participates in case management conferences and documents per guidelines.
* Schedules patients independently according to the individual plan of care.
* Attends staff meetings, in-services, other required meetings.
* Compliance with all mandatory requirements.
* Knowledge of accurate OASIS documentation.
Job Type: Full-Time, Part-Time, and Per diem available

Experience:
* Nursing: 2 years (Preferred)
* Home Care Experience (Preferred)
License/Certification:
Connecticut RN License (Required)
