What are the responsibilities and job description for the Physical Therapist position at Visiting Nurses of the Lower Valley, Inc?
Immediate per diem or part-time opening for experienced Physical Therapist in Essex and surrounding areas. Responsible for evaluating, developing a plan of care and providing therapy care to patients in coordination with physician orders. Performs initial and on-going assessment to determine level of functioning and prepares timely and appropriate clinical assessment and progress notes in accordance with accepted documentation standards. Candidates should be a graduate of an accredited college or university, licensed in the state of CT and have one year experience as PT in home care setting.
