As a QAPI and Compliance Coordinator, you’ll be on an interdisciplinary team that delivers life-changing care to patients across Suffolk County.
Your Impact:
Responsible for oversight and compliance with rules and regulations as per CMS Conditions of Participation, NYS DOH regulations, and CHAP Standards of Excellence for Certified Hospice.
You Will:
Review sample patient records for completeness and compliance with Hospice regulations.
Oversee and monitor the adoption, implementation and maintenance of the Compliance Plan for our Certified Home Health Agency (CHHA) and Hospice.
Review and respond to Medicare and other payment denials through submission of appeals up to and including Administrative Law Judge action as top level of appeal in the process.
Access CDC and DOH web resources to disseminate new information and directives as apply to Hospice.
Review Annual QAPI Plan as applies to Hospice including review of outcomes of previous year’s plan and proposal of goals to be incorporated in subsequent projected Annual QAPI Plan.
Maintain, review, and submit infection control and incident logs to identify need for related QAPI Project and/or staff education in coordination with Hospice Educator.
Review and respond to all requests for copies of clinical documentation, and record if needed as requests for PHI.
Apply and submit CHAP accreditation renewals, including review and updating of related policies.
Initiate and oversee audits necessary to fulfill goals of Plan of Correction.
Ensure timely submission of HIS data to CMS.
What You Bring:
Bachelor's degree in nursing, Master's degree in related field also preferred
Current Licensure to practice as a Registered Nurse in New York State
Three years Home Care and/or Hospice experience
Familiarity with Home Care and/or Conditions of Participation
Experience in budget and finance management related to health care
Skilled in communication and organizational management
Commitment to the Hospice philosophy and VNSHS agency mission
Adept computer skills and related understanding of software applications
