As a RN Intake Coordinator, you’ll work on an interdisciplinary team that delivers life-changing care to patients across Suffolk County.
Qualifications:
Graduation from a college or university accredited by the Council on Social Work Education with a master’s degree in social work.
Must be registered to practice in NYS as a licensed master’s social worker or as a licensed clinical social worker.
One year experience in acute care setting.
Experience in a certified home health agency.
You Will:
Referral Management: Receive, review, and process referrals for hospice services
Evaluate referrals to determine eligibility and appropriateness of services based on clinical, functional, and payer requirements.
Serve as a liaison between referral sources, patients, families, and agency staff Obtain and verify physician orders, demographic information, insurance coverage, and required documentation.
Coordinate services, equipment, supplies, laboratory services, and other resources needed to support patient care.
Ensure accurate and timely documentation within the electronic medical record while maintaining compliance with payer and regulatory requirements.
Referral Source Relations Build and maintain positive relationships with referral sources.
Provide timely communication and updates regarding referral status.
Serve as a resource regarding hospice, home health, and community-based services Participate in educational and outreach activities that support community awareness and referral engagement.
Care Coordination and Agency Collaboration Collaborate with clinical and operational teams to facilitate timely admissions and continuity of care.
Communicate referral status, staffing needs, and admission updates to appropriate agency personnel Monitor referral activity and maintain required statistical data and reports.
Participate in performance improvement initiatives, staff orientation, and training programs Professional Standards Represent VNSHS professionally and support the organization's mission and values Maintain compliance with all federal, state, local, and organizational policies and regulations Adhere to HIPAA and confidentiality requirements Perform other related duties as assigned in support of agency operations and patient care.
