Licensed Practical Nurse (LPN) - Per DiemVisiting Nurse & Hospice of Litchfield County is seeking experienced Per Diem LPNs to join our clinical teams serving Litchfield County and the Greater Farmington Valley. Must have availability for weekends and one holiday per year.Duties include performing home visits; observe, document, and report patient status; and coordinate care with multi-disciplinary team. The Agency utilizes Netsmart for its EMR and follows National Best Practice Guidelines for caseload and visit productivity.QUALIFICATIONS:Current license as a Licensed Practical Nurse in ConnecticutValid driver’s licenseReliable transportation and willing to travel inStrong communication and interpersonal skillsMinimum 1- year Home health & Hospice experienceEmail resume to: jhall@vnhlc.org
