Are you a compassionate, strategic leader with a passion for end-of-life care? We are seeking an experienced Hospice Program Director to lead and oversee our hospice program, ensuring the delivery of high-quality, patient-centered care while maintaining operational excellence.

Exceptional time management and organizational skills are essential, as this role involves coordinating interdisciplinary teams, managing compliance, overseeing budgets, and ensuring timely service delivery.
Key Responsibilities:
Provide leadership and direction to clinical and administrative hospice staff
Ensure compliance with state and federal regulations
Leads and supervises the Interdisciplinary Team (IDT)
Works closely with Hospice Medical Director and Hospice Physicians
Oversees patient care plans, staff scheduling, and resource allocation
Monitor performance metrics and implement quality improvement initiatives
Facilitate policy and procedure changes to enhance program development
Foster a culture of compassion, teamwork, and accountability
Acts as a liaison within the community to the agency's referral services.
Qualifications:
RN, licensed in the State of Connecticut, preferably a BSN, or a registered nurse with an advanced degree in a related field. An MSN or master's level preparation is a plus.
Meet the qualifications of nurse supervisor of clinical services in Sec. 19-13-D67 of home health care agency licensure regulation.
Minimum 3 years of hospice or palliative care experience, with at least 2 years of which were in community health.
Deep understanding of hospice regulations and standards of care
Demonstrated ability to lead teams, manage complex schedules, and meet deadlines
Excellent communication, interpersonal, and decision-making skills.
