Hospice Medical Social Worker – Part-time
Visiting Nurse Health System (Visiting Nurse) is seeking a Medical Social Worker with experience in the Home Hospice space to cover our Covington and Stone Mountain area. The ideal candidate provides social, emotional, and psychological services/counseling to terminally ill patients and their bereaved families, enabling the patient to maintain independence and remain at home. The Hospice Medical Social Worker will facilitate placement in an appropriate facility for alternative care requested by the patient/family.

In this position...
- Demonstrates the skill and knowledge in counseling patients/ families using a variety of clinical theories of short- & long-term interventions.
- Demonstrates sensitivity and skill in discussing loss issues with both children and adults.
- Demonstrates ability to integrate information and make sound clinical decisions with thorough follow-through.
- Demonstrates the skill to assess the patient/family in identifying social, emotional, financial & environmental needs/issues and develop an appropriate intervention plan.
- Demonstrates skill to assess the patient /family’s coping ability, e.g., strengths, weaknesses, and socio-cultural environment, to develop a sound base for an instructional plan.
- Demonstrates skill, sensitivity, and respect in working with patients/families of different cultures, ethnic backgrounds, and belief systems. Ensures equal access to services for all patients/families.
- Demonstrates knowledge of agency policies and procedures related to social work to provide services within approved social worker parameters.
- Demonstrates knowledge of eligibility criteria for federal and state services such as Medicare, Medicaid, AHS, SSI, Welfare, Veterans benefits, etc. Assists patients/families to receive benefits to which they are eligible.
- Demonstrates knowledge of community resources and facilitates appropriate referrals to assist patient/family.
- Demonstrates knowledge of counseling techniques and provides adequate counseling to patients/families.
Do you have....
- Master’s degree in Social Work from an accredited institution of higher learning
- GA licensure; LCSW preferred
- Must have reliable transportation, a valid driver's license, and current vehicle insurance
- Minimum of 1 year of experience
Compensation range: $63,280 - $94,921
