Hospice Admissions Nurse - Floater - 3 PM - 11 PM Shift
Visiting Nurse Health System (Visiting Nurse) is seeking a Hospice Admissions Nurse for a 3:00 – 11:00 PM shift. Hospice care is a special need for those approaching the end of their life journey. VNHS needs a calm, compassionate, and empathetic clinician who cares for the patient and their family during this difficult time. We're seeking an Admissions Nurse who is experienced in the Hospice space, knows this role is a calling on their life, understands the importance of patient care, and ensures every patient is treated with dignity, comfort, and care. We are looking for someone skilled, self-motivated, enthusiastic, and energetic, and willing to be the best in class in the Atlanta and Metro-Atlanta area. In this position...
- Provides nursing services as required per the skills competency checklist.
- Assesses patients for admission to the Hospice Atlanta Program. Admits patients for care based on skilled need and payor source eligibility requirements.
- Assesses & coordinates the need for additional services to enhance the patient’s care.
- Coordinates care between home, outpatient, and inpatient settings. Completes transfer form and initiates inpatient log for medical patients.
- Makes referrals to other agencies on the patient's behalf.
- Evaluates the patient’s response to care as evidenced by visit note documentation.
- Provides education to the patient & family regarding the disease process, self-care techniques, and prevention strategies, as evidenced by visit note documentation.
- All Clinical documentation is complete and accurate and submitted to the office per policy and guidelines.
- Participates in on-call rotation per program requirements.
- Demonstrates knowledge of infection control standards and techniques as per VNHS policy and procedures, and consistently practices these techniques.
Do you have....
- An Accredited Diploma, Associate Degree, or Baccalaureate School of Nursing.
- 3 years in Home Hospice who meets the standards for coordination of care and documentation
- Current licensure by the Georgia State Board of Nursing and valid CPR certification.
- Must have reliable transportation, a valid GA driver's license, and current vehicle insurance
Compensation Range: $72,248 - $108,372
