Physical Therapist

Responsibilities:
- Develop and implement physical therapy plans of care ensuring quality and safe delivery of physical therapy home health care services.
- Work directly with patients to achieve maximum physical recovery.
- Evaluate effects of therapy treatment and communicate patients' progress.
- Record and document patient care services.
- Promote education for clients, client families and the community.
- Coordinate provision of physical therapy home health care services that reflect VNCC’s philosophy and standards of home health for assigned clients
Qualifications:
- Current PT licensure in MA required.
- Two (2) years experience in home health care, elder services, and/or public health preferred.
- Strong assessment skills needed.
- Excellent communication skills with a customer service orientation necessary.
- Ability to build rapport with patients.
- Ability to handle physical workload.
- Work with a friendly and supportive team of seasoned clinicians at an agency that understands the need for work/life balance.
