Home Health Clinical Liaison
The Home Health Clinical Liaison, RN plays a key role in promoting home health services, building strong referral relationships, and ensuring smooth patient transitions to home care. This position combines clinical assessment skills with marketing and relationship development to increase referrals and strengthen the agency’s presence in the community.

Build and maintain strong relationships with physicians, hospitals, SNFs, clinics, and community partners to increase home health referrals.

Conduct 25–35 weekly outreach contacts to promote services and educate referral sources.

Complete clinical assessments in hospitals, SNFs, clinics, and other settings to determine eligibility for home health services.

Facilitate smooth patient transitions by collecting referral documentation and coordinating with physicians and discharge planners.

Serve as the primary point of contact for referral sources, providing timely communication and resolving issues that impact patient care or satisfaction.

Track referral activity, maintain accurate outreach records, and prepare weekly summaries of marketing activities.

Provide educational presentations to physicians, facilities, and community partners on home health services and agency capabilities.

Represent the agency at community events, health fairs, and professional gatherings to strengthen community presence.

Qualifications
- Graduate of an accredited school of nursing.
- Current Registered Nurse (RN) licensure in the state of Indiana.
- Strong attention to detail with a commitment to providing excellent customer service.
- Minimum of one year of experience in a healthcare setting required; business/marketing experience preferred.
- Prior experience in home care, hospice, or geriatrics strongly preferred.
- Proficient in Microsoft Office applications, including Word, Excel, PowerPoint, and Access.
- Must meet all pre-employment requirements.
