What You'll Do
As a Home Health Registered Nurse, You Will
- Build trusting relationships with patients and their families through compassionate, individualized care.
- Perform comprehensive nursing assessments and identify patient needs.
- Develop, implement, and revise personalized plans of care.
- Administer medications, treatments, wound care, and other physician-ordered services.
- Monitor patient progress and communicate changes in condition to physicians and the healthcare team.
- Teach patients and caregivers how to manage illnesses, medications, treatments, and safety concerns at home.
- Coordinate care with physicians, therapists, social workers, aides, and other members of the interdisciplinary team.
- Maintain accurate, timely, and compliant clinical documentation.
- Provide guidance and supervision to Licensed Practical Nurses (LPNs) and Home Health Aides as appropriate.
- Participate in on-call coverage as assigned.
- Ensure care is delivered in accordance with agency policies and all state and federal regulations.
We're Looking For Nurses Who Are
- Compassionate and patient-focused
- Self-motivated and comfortable working independently
- Excellent communicators and critical thinkers
- Organized and able to prioritize multiple responsibilities
- Passionate about helping individuals remain safely at home
- Current Registered Nurse (RN) license in the state of employment
- At least one year of nursing experience preferred
- Home health experience is welcomed but not required-we're happy to train the right nurse
- Valid driver's license
- Reliable transportation
- Current automobile insurance
