Schedule: Monday through Friday | 12:00 PM – 8:00 PM

The VNA of Hanover & Spring Grove is seeking a dedicated and compassionate Registered Nurse to join our interdisciplinary team in a dual-capacity role supporting both Home Health and Hospice services. This position offers a unique opportunity to provide skilled care during daytime home visits while also responding to emergent Hospice needs during evening hours.

Position Overview:

The Home Health and Hospice RN will manage a small Home Health caseload from 12:00 PM to 4:00 PM, followed by providing Hospice support from 4:00 PM to 8:00 PM. Evening responsibilities include completing hospice consents, administering IV medications, and responding to urgent patient needs. The ideal candidate will demonstrate strong clinical judgment, excellent communication skills, and the ability to work independently while collaborating within a team-oriented environment.

Key Responsibilities:

Clinical Care & Case Management

• Perform comprehensive nursing assessments and develop individualized care plans

• Provide after-hours hospice services including consents and IV administration

• Deliver patient and caregiver education on disease progression and symptom management

• Monitor and report changes in patient conditions and adjust care accordingly

• Collaborate with physicians and interdisciplinary teams for coordinated care delivery

Documentation & Communication

• Maintain timely, accurate clinical documentation in the electronic health record (EHR)

• Communicate professionally with patients, families, and team members

• Participate in care conferences and case reviews

Efficiency & Leadership

• Effectively manage caseloads and prioritize care delivery

• Serve as a role model for professionalism and teamwork

• Supervise Home Health and Hospice Aides as required

• Maintain current PA licensure and CPR certification

Qualifications:

• Graduate of an accredited nursing program

• Current RN license in the State of Pennsylvania

• Minimum of one (1) year of nursing experience; Home Health and/or Hospice experience preferred

• Strong clinical assessment, organizational, and documentation skills

• Valid PA driver’s license, reliable transportation, and current auto insurance

Working Conditions:

• Ability to work in a variety of home and community settings

• Must be able to lift up to 50 pounds and manage physical demands of home care

• Must be comfortable working independently and handling urgent clinical situations
