Are you passionate about helping patients regain independence in the comfort of their homes? The VNA of Hanover & Spring Grove is seeking a dedicated Occupational Therapist to join our Home Health team. This role plays a vital part in delivering exceptional, patient-centered care and supporting our mission to provide high-quality home-based services with integrity, respect, teamwork, and excellence.
What You’ll Do:
Evaluate patients’ functional status and home environments
Create individualized treatment plans focused on daily living skills and independence
Educate patients and caregivers on assistive devices and energy conservation
Collaborate closely with physicians, nurses, therapists, and caregivers
Provide palliative therapy services for hospice patients as needed
Ensure documentation and care meet Medicare and state regulations
What We’re Looking For:
Graduate of an accredited Occupational Therapy program
Licensed Occupational Therapist in Pennsylvania
At least 2 years of OT experience (home health preferred)
Valid driver’s license, reliable transportation, and insurance
CPR certification and clean background check
