We are currently looking for a Registered Nurse Field Manager to join our team in Essex County. This position would be responsible for planning, directing and supervising the quality of care provided by the Home Health Nurses and providing back up support to the Clinical Manager as needed.  Responsible for providing coordination of internal and external services for patients on the team.

Responsibilities

Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers

Facilitates and coordinates the case management, interdisciplinary care and services provided by the staff nurses to patients and families

Makes field visits with Home Health Nurses to client’s homes

Monitors the quality and appropriateness of services provided by the staff

Provides back-up support to the Clinical Manager as needed

Participates in VNAHG’s QAPI and in-service programs

Adheres to agency infection control and safety policies

Adheres to the organization’s policy in regards to absenteeism and appearance

Supervises the Nursing staff in conjunction with the organization’s managerial and leadership practices

Provide orientation to new staff in conjunction with Preceptor and Clinical manager

Provide ongoing education to clinicians in conjunction with Education Department

Qualifications

RN with Bachelor’s degree in Nursing required or in progress

2 years of experience in community health nursing providing direct patient care

Strong computer skills   

Excellent organizational and interpersonal skills with ability to supervise others

Excellent verbal and written communication skills; bilingual preferred 

Working Conditions/Physical Demand: Business Office Environment with phone and computer use
